Job Summary
Assess assigned patients, establish and implement the Plan of Care, and collaborate with pharmacy staff, providers, and caregivers to monitor therapy goals. Manage vascular access devices, including insertion of peripheral infusion devices and venipuncture for labs, while educating patients on complex pharmaceutical products. Complete patient care documentation at the patient's home or AIS, coordinate care with home health agencies, and participate in the scheduled on-call rotation. Track mileage expenses monthly and complete annual competencies and mandatory training. Requires a valid driver's license and 100% regular driving to patient homes for visits.
Required Qualifications
- Graduate of an accredited school of nursing
- Bachelor of Science in Nursing
- Valid nursing license in state(s) of practice
- Minimum of one (1) year of experience as a Registered Nurse
- Current CPR certification
- Valid driver's license and auto insurance
- Must meet company standards for driving record as well as driver age requirement
- Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
- The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs
Desired Qualifications
- Previous infusion and home health care experience preferred
- Extensive experience in IV therapy preferred
- Flexible schedule with the availability to work a variety of shift models
- Ability to work effectively within a multidisciplinary team
- Effective verbal and written communication skills
