Job Summary
Assess assigned patients, establish and implement Plans of Care, and collaborate with pharmacy staff, providers, and caregivers to monitor therapy goals. Manage vascular access devices, including peripheral infusion insertions and venipuncture, while completing patient care documentation at the patient's home or AIS. Educate patients on medication administration and coordinate nursing care with home health agencies as indicated. Participate in on-call rotation per branch protocol and complete monthly expense reports tracking mileage. Maintain annual competencies and mandatory training within assigned due dates. Drive to patient homes for visits in the Mohave Valley territory, adhering to all safety and regulatory standards.
Required Qualifications
- Graduate of an accredited school of nursing
- Bachelor of Science in Nursing
- Valid nursing license in state(s) of practice
- Minimum of one (1) year of experience as a Registered Nurse
- Current CPR certification
- Valid driver's license and auto insurance
- Must meet company standards for driving record as well as driver age requirement
- Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
- **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
Desired Qualifications
- Previous infusion and home health care experience preferred
- Extensive experience in IV therapy preferred
- Flexible schedule with the availability to work a variety of shift models
- Ability to work effectively within a multidisciplinary team
- Effective verbal and written communication skills
