Job Summary
Assess assigned patients, establish and implement their Plan of Care, and collaborate with pharmacy, providers, and caregivers to monitor therapy goals. Manage vascular access devices, including insertion of peripheral infusion devices and venipuncture, while educating patients and caregivers on their conditions. Complete patient care documentation at the patient's home or AIS, evaluate progress toward goals, and recommend physician changes as needed. Coordinate nursing care with home health and community agencies, participate in on-call rotation, and complete monthly expense reports tracking mileage. Adhere to INS, IgNS, CDC, and state regulations regarding scope of services and standards of practice. Requires valid driver's license, ability to drive 100% of visits to patient homes, and flexibility for day, evening, and weekend shifts.
Required Qualifications
- Graduate of an accredited school of nursing
- Bachelor of Science in Nursing
- Valid nursing license in state(s) of practice
- Minimum of one (1) year of experience as a Registered Nurse
- Current CPR certification
- Flexible schedule with the availability to work a variety of shift models
- Ability to work effectively within a multidisciplinary team
- Effective verbal and written communication skills
- Valid driver's license and auto insurance
- Must meet company standards for driving record as well as driver age requirement
- Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
- The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs
Desired Qualifications
- Previous infusion and home health care experience preferred
- Extensive experience in IV therapy preferred
