Job Summary
Assess assigned patients, establish and implement their Plan of Care, and collaborate with pharmacy staff, providers, and caregivers to develop therapy goals. Manage vascular access devices, including peripheral infusion device insertion and venipuncture, while completing patient care documentation at the patient's home or AIS. Educate patients and caregivers on medication administration and evaluate progress toward therapeutic goals to recommend physician changes. Coordinate nursing care with home health agencies and participate in the scheduled on-call rotation. Complete monthly expense reports tracking mileage and mandatory annual competencies. Drive to patient homes in Pima County for 100% of visits, adhering to safety standards and physical requirements for lifting and prolonged sitting or standing.
Required Qualifications
- Graduate of an accredited school of nursing
- Bachelor of Science in Nursing
- Valid nursing license in state(s) of practice
- Minimum of one (1) year of experience as a Registered Nurse
- Current CPR certification
- Valid driver's license and auto insurance
- Must meet company standards for driving record as well as driver age requirement
- Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
- **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
Desired Qualifications
- Previous infusion and home health care experience preferred
- Extensive experience in IV therapy preferred
- Flexible schedule with the availability to work a variety of shift models
- Ability to work effectively within a multidisciplinary team
- Effective verbal and written communication skills
