Job Summary
Assess assigned patients and implement their Plan of Care while adhering to INS, IgNS, CDC, and state regulations. Manage vascular access devices, including insertion of peripheral infusion devices and venipuncture, and educate patients and caregivers on therapy. Collaborate with pharmacy staff, providers, and patients to develop and monitor treatment goals, then evaluate progress and recommend physician changes. Complete patient care documentation at the patient's home or AIS, coordinate care with home health agencies, and participate in the on-call rotation. Track mileage for expense reports and complete annual competencies and mandatory training. Requires a valid driver's license, current CPR certification, and ability to drive 100% of visits to patient homes.
Required Qualifications
- Graduate of an accredited school of nursing
- Bachelor of Science in Nursing
- Valid nursing license in state(s) of practice
- Minimum of one (1) year of experience as a Registered Nurse
- Current CPR certification
- Valid driver's license and auto insurance
- Must meet company standards for driving record as well as driver age requirement
- Percentage of Travel: 100%-Regular driving to patient's homes for visits is required as part of this position
- **To perform this role will require constantly sitting, standing, walking, bending, reaching, crouching, stooping and typing on a keyboard with fingers. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
Desired Qualifications
- Previous infusion and home health care experience preferred
- Extensive experience in IV therapy preferred
- Flexible schedule with the availability to work a variety of shift models
- Ability to work effectively within a multidisciplinary team
- Effective verbal and written communication skills
