Job Summary
Conduct initial physical assessments and history reviews for homebound clients to determine care needs. Initiate and revise plans of care, administer prescribed medications, and provide direct nursing procedures per the State Nurse Practice Act. Develop therapeutic, preventive, and rehabilitative care plans with patient and family input, while counseling on health instructions and discharge planning. Coordinate patient care as a Case Manager when assigned, communicate with physicians regarding condition changes, and prepare clinical notes every 60 days. Supervise home health aides biweekly, ensure equipment availability, and participate in on-call rotations. Requires current RN licensure, CPR certification, and ability to lift patients; pay is per visit ($50–90) with full-time and PRN options available.
Required Qualifications
- Graduate of an accredited school of nursing
- One (1) to two (2) years of recent acute care experience in an institutional setting
- Current licensure in state
- CPR certification
- Bachelor's degree
- Licensed driver
- Automobile that is in good working order
- Insured automobile in accordance with the organization requirements
- Reliably commute or planning to relocate before starting work
- RN-Registered License
