RN - Registered Nurse - Hospital Case Manager

Assess, plan, implement, and coordinate patient services to optimize health status while managing utilization metrics to refine care delivery models. Integrate evidence-based guidelines into treatment plans that promote quality and efficiency, and develop systems to monitor progress and promote early intervention in acute care situations. Design individualized care plans by collaborating with patients, families, primary care providers, and the broader healthcare team to ensure service coordination. Continuously evaluate laboratory results and diagnostic tests to adjust treatment plans and maintain required documentation for all case management activities.
