RN - Registered Nurse - Hospital Case Manager

RN Hospital Case Manager on Geisinger’s Care Coordination team; assess, plan, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient or member's health status. Manage utilization and practice metrics to refine the delivery of care and maximize clinical, quality, and fiscal outcomes. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of patient-centric treatment plans, promoting quality and efficiency in care delivery for the identified population. Develops systems of care that monitor progress and promote early intervention in acute care situations. Collaborates with the patient or member, family, primary care provider, and other health care team members to ensure coordination of services. Works to apply benefits and utilization management while maintaining required documentation. On-site role at Geisinger Medical Center with typical schedule Monday–Friday 8:00 AM to 4:30 PM, with occasional weekend coverage as part of a shared rotation.
