RN / Registered Nurse - Hospice Admissions

Visit patients in homes, hospitals, and nursing facilities referred for hospice care. Explain services, discuss goals, obtain consents, and perform clinical assessments to determine eligibility based on Medicare/insurance guidelines. Gather medical history, medications, and symptoms, then coordinate the start of nursing, aide, equipment, and medication services. Collaborate with the interdisciplinary team and maintain accurate documentation. Requires one year of hospice experience (3+ preferred) and a licensed driver with an insured vehicle. Schedule includes 9-6 or 10-7 with alternating weekends.
