RN - Registered Nurse - Home Visitor - Community Based - Part Time

Assess physical, emotional, social, and environmental needs of women and their families enrolled in the Nurse Family Partnership. Complete home visits in Northumberland County and other community settings, providing guidance on pregnancy, infants, and toddlers based on standardized protocols. Initiate plans of visitation, establish goals, and evaluate outcomes while completing documentation within 48 hours. Provide discharge planning, educate referral sources, and develop therapeutic relationships using motivational interviewing. Work part-time, 20 hours weekly from 8 am to 4:30 pm with occasional evening flexibility.
