Assist in developing, reviewing, and revising the plan of care while providing nursing care according to physician orders and individualized treatment plans. Observe, record, and report client reactions to treatment and condition changes to appropriate personnel or physicians. Provide ongoing instruction and supervision to clients, families, or other health team personnel regarding the treatment plan. Monitor and record patient conditions, documenting provided care services via electronic charting supported by AI tools. Participate in education programs, quality assessment activities, and staff meetings as required. This role involves skilled nursing visits in Topeka, KS and surrounding areas, working alongside physical, speech, and occupational therapists. We offer flexible full-time, part-time, and PRN schedules with competitive pay ranging from $65,000 to $90,000, paid mileage, and comprehensive benefits including medical, dental, vision, and 401K.
