(RN) Registered Nurse - Home Health Per Diem

Conduct home health visits to administer services, perform comprehensive assessments, and update physician orders. Administer IVs, change dressings, clean wounds, and oversee patient treatment plans while documenting all activities in Synergy Software. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, coordinating with physicians and other team members regarding patient condition changes. Supervise Home Health Aides per regulatory requirements and manage re-certifications, transfers, and discharges. Requires current CA RN license, BLS certification, IV/Phlebotomy skills, and one year of clinical experience. Work seven days a week with flexibility for scheduling, including weekends and holidays.
