Job Summary
RN Registered Nurse Home Health Clinical Director leads and directs the home health department at Deer Meadows Rehabilitation Center, supervising home health nurses, providing clinical guidance, developing and implementing patient care plans, ensuring regulatory compliance and accreditation standards, facilitating interprofessional collaborations, analyzing clinical data to identify improvement opportunities, maintaining patient care records (EMR), and coordinating education/training programs with other departments to ensure high-quality patient care and satisfaction in Philadelphia, PA.
Required Qualifications
- Current RN license in Pennsylvania
- Minimum 2-3 years experience in home health or related field
- 1-2 years in leadership/management role
- Strong clinical knowledge in home health care
- Excellent communication, leadership, and organizational skills
