Manage a caseload of home health patients, supervising LPN visits and coordinating with Physical Therapy, Occupational Therapy, and Speech Therapy teams. Conduct assessments, build personalized care plans aligned with physician orders, and support patients managing conditions like CHF, COPD, diabetes, wounds, and medication regimens. Earn $78,000–$88,000 base plus a $10,000 sign-on bonus under a pay-per-visit model. Work a real Monday–Friday schedule with on-call once monthly and build your own visit schedule for autonomy. Complete 4–6 weeks of paid training with guaranteed pay. See a ride-along before committing. Phoenix Home Care and Hospice offers PTO accruing from day one and reimburses mileage on every visit.
Required Qualifications
