Job Summary
RN Home Health Case Manager responsible for hands-on patient care in patients' homes, coordinating services of other healthcare providers, communicating with health insurers, and overseeing visiting nurses. Requires strong skills in physical assessment, diagnostic interpretation, and developing personalized, diagnosis-driven care plans aligned with physician orders; collaborates with PT/OT/Speech and education on health conditions (CHF, COPD, Diabetes, Wound Care, Medication Management). Works with physicians to obtain orders and develop care plans; utilizes AI-powered charting during visits to support balance between patient care and documentation. Located in St. Louis, MO with full-time schedule Monday–Friday, days; offers comprehensive benefits and a sign-on bonus.
Required Qualifications
