RN / REGISTERED NURSE - HOME HEALTH - CASE MANAGER

Perform comprehensive patient assessments during home visits, evaluating physical, emotional, and environmental needs. Deliver skilled nursing care in accordance with physician orders, including PleurX drain management, wound care, IV administration, and telehealth monitoring. Educate clients and caregivers on disease management, medications, and self-care techniques while building meaningful relationships to support patient independence. Collaborate with an interdisciplinary team of physicians, therapists, and social workers to coordinate care and communicate changes in patient status promptly. Complete timely OASIS documentation in accordance with regulatory standards. Requires one year of recent home health experience, RN licensure, BLS certification, and valid driver's license. Full-time role with competitive hourly pay ranging from $36.20 to $59.73.
