RN / Registered Nurse - Home Health

Assess and monitor the physical, emotional, and psychological needs of patients while creating home health care plans that align with medical orders and patient goals. Direct nursing care by administering medications, treatments, and interventions, and provide pain and symptom management. Educate and support the patient's family and caregivers, collaborate with an interdisciplinary team, and maintain accurate and timely documentation. Participate in on-call rotation as required by the local branch. This role offers full-time flexibility with a focus on 1:1 care, reduced time on feet, and team support within a growing, community-focused organization.
