Job Summary
Provide compassionate case management for patients in their final days, building trusting relationships with families and serving as the central communication point for the interdisciplinary team. Deliver medical assistance while honoring individual wishes and ensuring dignity and comfort during the patient's journey. Maintain active unencumbered state RN licensure with at least six months of prior skilled nursing experience. Work Monday through Friday from 8:30 AM to 5:00 PM with no mandatory on-call rotation. Required Qualifications
- 6 months minimum prior skilled nursing experience
- An active unencumbered state Registered Nurse license
- Current driver's license and auto liability coverage
- Interest and ability in providing patient care in homes and facilities
