Coordinate disease management services and provide primary clinical support to providers, patients, and families for diagnosis and treatment. Assess healthcare, educational, and psychosocial needs to establish treatment plans, while investigating options and facilitating communication among the care team. Implement evidence-based clinical interventions, adjust therapies as directed, and monitor outcomes through follow-up. Manage preventative care by coordinating laboratory and diagnostic tests, collecting specimens, and administering medications and vaccines under provider direction. Maintain required documentation in EPIC, prepare laboratory paperwork, and participate in professional development and literature reviews. Serve as a resource for patients and families regarding disease management processes and outcomes.
