Job Summary
Administer community-based strategies to improve health through education, screening, and evidence-based programming for chronic and complex patients. Utilize the nursing process to assess, diagnose, and establish patient-centered plans across shelters and non-structured settings, including respite care as needed. Assist individuals in accessing specialized programs via screenings and referrals while maintaining relationships with local churches and service organizations. Recruit and train community volunteers, organize flu shot clinics, and link patients with tangible resources like schools and businesses. Work as an integral member of an inter-professional team to bridge care between primary visits and hospital admission.
Required Qualifications
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
- Obtains and subsequently maintains required department specific competencies and certifications
Desired Qualifications
- Bachelor's degree in nursing preferred
- Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
