Job Summary

Conduct nursing assessments, administer medications and treatments, and manage chronic conditions for older adults with complex medical needs within the PACE clinic. Coordinate treatments and specialty appointments, maintain accurate clinical documentation, and ensure compliance with regulatory standards. Educate participants and caregivers on disease management and healthy living while communicating with physicians and the interdisciplinary care team. Respond to acute health concerns and advocate for participants to prevent unnecessary hospitalizations. Requires current Kansas RN license, 1-2 years clinical experience, BLS certification, and proficiency with electronic health records.

Required Qualifications

  • Current, unrestricted Registered Nurse (RN) license in the state of Kansas
  • Graduate of an accredited school of nursing
  • Minimum of 1-2 years of clinical nursing experience, preferably in geriatrics, primary care, long-term care, home health, hospice, or a similar setting
  • Current BLS (Basic Life Support) certification
  • Strong clinical assessment, critical thinking, and care coordination skills
  • Excellent communication and interpersonal skills with the ability to build trusting relationships with older adults, caregivers, and interdisciplinary team members
  • Ability to prioritize multiple responsibilities in a fast-paced clinical environment
  • Proficiency with electronic health records (EHR) and basic computer applications
  • Valid driver's license, reliable transportation, and an acceptable driving record

Desired Qualifications

  • Bachelor's degree in Nursing (BSN)
  • Experience working in a PACE program or with frail older adults and individuals with multiple chronic conditions
  • Experience with interdisciplinary team-based care and care planning
  • Knowledge of Medicare, Medicaid, and PACE regulations
  • Experience providing patient and caregiver education related to chronic disease management and preventive care
