Job Summary
Conduct comprehensive assessments of patients and families to determine home care needs, including physical exams and history taking. Initiate and revise plans of care with physicians, administering medications and implementing preventive or rehabilitative procedures. Perform high-tech nursing skills such as catheter management, ostomy care, and wound therapy while ensuring infection control protocols. Counsel patients and families on health instructions, coordinate care with specialists, and supervise home health aides or LPNs. Prepare clinical notes and submit documentation to physicians at least every sixty days. Participate in on-call duties as defined by policy and ensure availability of necessary equipment and supplies.
Required Qualifications
- Bachelor's degree of Nursing from an accredited college/university
- Current active and unrestricted nursing license
- Pediatric care experience in the acute and/or home care environment
- Valid driver's license
- Current certification in Basic Life Support for Health Care Professionals
- Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist
- Basic knowledge of computer operating systems and software applications to communicate and document care
Desired Qualifications
- Prior experience working with the Adults/ Geriatric population
- Special Needs population experience
- Pediatric experience (in addition to required acute/home care pediatric experience)
