Job Summary
Conduct comprehensive client assessments covering medical, psychosocial, emotional, cognitive, and safety needs while developing and revising individualized care plans. Coordinate services with physicians, psychologists, and interdisciplinary teams, participating in case conferences and discharge planning. Educate clients and caregivers on disease processes, medications, safety precautions, and home healthcare procedures. Administer medications, IV therapies, and lab draws as ordered, and supervise LPNs and paraprofessional staff. Maintain accurate clinical documentation and ensure compliance with state and federal regulations. Requires Maryland RN license, two years of home health experience preferred, and current CPR certification. Full-time and part-time roles available in Baltimore, MD.
Required Qualifications
- Graduate of an accredited School of Nursing
- Registered Nurse licensed by the state of Maryland
- Criminal background check
- Current CPR certification
- Reliable transportation and valid driver's license
Desired Qualifications
- Minimum of two (2) years of experience as a professional nurse in a Home Health Agency
