RN / Registered Nurse Case Manager - Hospice

Assess and monitor the physical, emotional, and psychological needs of patients to create hospice care plans aligned with their wishes. Administer medications, treatments, and interventions while providing pain and symptom management. Educate and support patient families and caregivers, then collaborate with the interdisciplinary team. Maintain accurate and timely documentation and participate in on-call rotation as required by the local branch. This role offers M-F 8a to 5p scheduling with a focus on 1:1 care in a supportive environment where you can work independently with team support.
