RN / Registered Nurse Case Manager - Hospice

Assess and monitor physical, emotional, and psychological needs of patients while creating hospice care plans aligned with their wishes. Administer medications, treatments, and interventions to manage pain and symptoms, then educate and support the patient's family and caregivers. Collaborate with the interdisciplinary team and maintain accurate, timely documentation. Participate in on-call rotation as required by the local branch. One year of nursing experience is required, with three years preferred, and a licensed driver's license is mandatory.
