RN / Registered Nurse Case Manager - Hospice

Assess and monitor the physical, emotional, and psychological needs of patients while creating hospice care plans that align with their wishes and goals. Administer medications, treatments, and interventions to manage pain and symptoms, then educate and support patient families and caregivers. Collaborate with the interdisciplinary team and maintain accurate, timely documentation of all care activities. Participate in the required on-call rotation as needed by the local branch. Full-time Monday-Friday 8AM-5PM schedule with travel required throughout the service area.
