RN / Registered Nurse Case Manager - Hospice

Assess and monitor physical, emotional, and psychological needs of patients while creating hospice care plans aligned with their wishes. Deliver direct nursing care including administering medications, treatments, and interventions, alongside pain and symptom management. Educate and support patient families and caregivers, then collaborate with the interdisciplinary team to ensure comprehensive support. Maintain accurate and timely documentation and participate in on-call rotation as required by the local branch. Full-time Monday-Friday 8AM-5PM schedule with travel required throughout the Sioux City territory.
