Job Summary
Assess and monitor physical, emotional, and psychological needs of patients while creating hospice care plans aligned with their wishes. Direct nursing care by administering medications, treatments, and interventions, and manage pain and symptoms. Educate and support patient families and caregivers, then collaborate with interdisciplinary teams including social work and chaplaincy. Maintain accurate documentation using Homebase EMR and participate in on-call rotation. Travel throughout Waukesha and Milwaukee counties for full-time Monday-Friday coverage.
Required Qualifications
- Registered Nurse
- Graduate of an accredited nursing school
- Current licensure to practice in state of operation
- One year nursing experience
- Licensed driver
- Automobile that is insured in accordance with state/or organization requirements
- Automobile in good working order
- Travel throughout the territory
- Primarily Waukesha and Milwaukee Counties
- Full Time Monday-Friday 8AM-5PM
- Participate in on-call rotation as required by local branch
Desired Qualifications
- 3+ years nursing experience
- Experience in hospice or a similar setting
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
