Job Summary
As a Home Health Case Manager, coordinate services, oversee visiting nurses, and communicate with health insurance providers to develop physician-order-driven care plans. Manage a varied caseload including routine visits, evaluations, and starts of care; perform strong physical assessments and diagnostic interpretation; educate patients on CHF, COPD, Diabetes, wound care, and medication management; collaborate with PT/OT/Speech therapies; and guide charting during visits using AI-powered support to maintain high-quality, patient-centered home health care.
Required Qualifications
- Missouri or Compact Registered Nursing and Driver's License.
