Job Summary
As a Registered Nurse Case Manager (RNCM) at Beaumont Home Health, you will lead and coordinate home-based nursing care for patients under an attending physician, performing OASIS assessments to develop individualized care plans, administering medications as prescribed, supervising nurse aides/LPNs, and educating patients and families to ensure understanding and safe care. You will work autonomously within state scope of practice, collaborate with an interdisciplinary team to minimize patient risk and reduce rehospitalizations, and participate in evaluating and updating plans of care to support patient recovery in the home environment.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
