RN (Registered Nurse) Case Manager

Job Summary
Oversee patient care from episode start to finish by updating doctor orders, administering prescribed medications, and performing OASIS assessments to develop individualized plans. Monitor patient conditions, counsel families on disease processes and preventative measures, and incorporate patient input into care planning to decrease re-hospitalizations. Supervise nurse aides and LPNs, evaluate treatments against the plan of care, and communicate supervisory visit results. Complete all clinical documentation, assessments, and education within 24 hours of visits per agency and Medicare guidelines. Drive to assigned locations and fulfill RN visits within federal and state specifications. Participate in on-call requirements, case conferences, and mandatory trainings while maintaining an active license.
Required Qualifications
- Minimum of one year of experience as an RN in an acute care setting
- Registered Nurse with current license in the state of employment
- Valid driver's license and auto insurance with your name as a listed driver
- Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers
- Ability to remain calm, have patience and be accommodating
- Compassionate and caring while working with patients
- Knowledgeable on nursing best practices
- Ability to make appropriate nursing judgments
- Ability to identify a situation and handle it with the best possible solution
- Detail-oriented and observant
- Disciplined style of work ethic with the ability to prioritize and be timely
- Ability to follow directions and work as a team member
