Job Summary
Provide direct patient care to a diverse population in post-acute rehabilitation and long-term skilled nursing. Develop individualized care plans, conduct assessments, and administer medications and therapies as prescribed. Collaborate with interdisciplinary teams, patients, and families to ensure continuity of care while documenting progress accurately. Participate in quality improvement initiatives to enhance patient outcomes and satisfaction. Requires current Pennsylvania RN licensure and ability to work in a fast-paced environment.
Required Qualifications
- Current RN licensure in the state of Pennsylvania
- Minimum of 0 years of experience in a long-term care or subacute setting
