Assure effective, outcome-focused nursing care through the nursing process while delivering and maintaining resident comfort. Identify discharge planning needs and provide input throughout the length of stay, collaborating with the resident, responsible party, and interdisciplinary team to develop individualized plans of care. Implement these plans consistently and cost-efficiently, sharing relevant information with physicians and other healthcare professionals to adjust care as needed. Maintain licensure in good standing with CPR certification obtained within 90 days of hire.
