Job Summary
Conduct comprehensive assessments of patients and families to determine home care needs, including physical assessments, history of illness, and response to therapy. Initiate and revise plans of care with physicians, administering medications and treatments as prescribed while counseling families on nursing needs. Perform high-tech nursing procedures such as catheter insertion, ostomy care, and negative pressure wound therapy, ensuring adherence to infection control and ethical guidelines. Supervise and evaluate home health aides and LPNs/LVNs per regulatory schedules, coordinate care with community providers and specialists, and prepare clinical notes for physician updates. Participate in on-call duties and manage equipment and supply arrangements. Maintain knowledge of pediatric nursing from birth through late teens and community health education.
Required Qualifications
- Bachelor's degree of Nursing from an accredited college/university
- Current active and unrestricted nursing license
- Valid driver's license
- Current certification in Basic Life Support for Health Care Professionals
- Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist
- Basic knowledge of computer operating systems and software applications to communicate and document care
- Prior experience working with the Adults/ Geriatric population
- Prior experience working with the Special Needs population
- Valid Registered Nurse (RN) license in the state of NY
- Experience in pediatric nursing from birth through late teens
