RN Quality Practice Advisor/Clinical Liaison

Job Summary
Coordinate performance improvement activities to engage clinical providers and manage initiatives that improve health outcomes for Healthmap Solutions members. Develop partnerships with practice locations to identify opportunities for process improvement in office workflows and provider performance regarding Healthmap Solutions Interventions, Care Gaps, and Chronic Conditions. Incorporate education on Best Practices, HEDIS measures, and topics related to Chronic Kidney Disease to support provider engagement. Partner with the Care Navigation team to identify members benefiting from support and maintain thorough documentation of meetings and interactions in compliance with NCQA standards. Collaborate with internal stakeholders to ensure timely delivery of reports and enhance care coordination. Requires a Bachelor's degree, active RN license, BLS certification, and 3+ years of healthcare or managed care experience. Heavy travel (over 50%) to support practices within the assigned geographic market.
Required Qualifications
- Bachelor's degree
- Active, unrestricted, RN license
- Basic Life Support (BLS) certification
- 3 years of experience in a health care or managed care setting
- 3+ years of progressive experience in healthcare services, clinical operations, quality, or care management
- Proof of valid and unrestricted driver's license
- automobile insurance as required by law
- Same state residency
- Current Tuberculosis (TB) test or current chest X-ray
- Proof of immunizations (e.g., Hepatitis B, MMR, Varicella, COVID-19, Influenza)
- Participation in annual health and wellness screenings
- Must be proficient in Microsoft Office: Outlook, Word, Excel, PowerPoint
- Heavy Travel, over 50%, to support practices in an assigned geographic market
Desired Qualifications
- 3 years of experience in claims or gap closure campaigns
- Prior experience building and managing relationships with health care providers
