RN Program Coordinator - Cardiology

Job Summary
Coordinate nursing care for cardiology patients from hospital discharge through follow-ups; facilitate patient plans of care, provide education and instructions for procedures and/or testing ordered by providers, review Health Risk Assessments, monitor charts for test results and reviews with providers and patients, manage preoperative surgical clearance requests with providers, and triage patient calls with health advice and referrals as appropriate. Requires RN license with ACLS/BLS; BSN preferred; EPIC experience preferred; 40 hours weekly, days shift, on-site in Oregon.
Required Qualifications
- Current State license as a Registered Nurse
- ACLS and BLS certification
- One year of relatable or equivalent experience
