RN (PRN)

Utilize the nursing process to plan and provide direct care to residents as assigned while leading shift routines and evaluating personnel activities in the assigned unit. Serve as the "Building Charge" when directed by the Director of Nursing, manage charting and documentation functions, and participate in facility committees to enhance interdisciplinary processes. This role requires current Colorado RN licensure and CPR certification, with long-term care experience preferred. Located in Delta County, Horizons Care Center offers a family-first approach to long-term care and rehabilitation services within a peaceful, small-town setting.
