RN (PRN)

Provide case management, coordination, supervision, and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act. Review client referral information, respond to requests and inquiries, and perform client assessments based on agency standards of practice to ensure effective and appropriate home care services. Direct, plan, and initiate appropriate action independently and responsively in-home care situations while performing job duties in accordance with agency policies, procedures, and professional standards. Perform other job duties as assigned by the Director of Clinical Services. This role supports the Medicare/Medicaid-certified Home Health of Western Colorado team in serving older adults across Delta, Montrose, and Ouray counties with compassion and empathy.
