Job Summary
Provide case management, coordination, supervision, and implementation of professional and supportive services to clients using nursing theory and process in accordance with the Nurse Practice Act. Review client referral information and respond to requests and inquiries as appropriate. Perform client assessments based on agency standards of practice to ensure effective and appropriate home care services. Direct, plan, and initiate appropriate action independently and responsively in-home care situations. Perform job duties in accordance with agency policies, procedures, and professional and community standards. Perform other job duties as assigned by the Director of Clinical Services. This role requires a current Colorado RN license, two years of nursing experience in long-term care, dementia care, or acute care, and CPR certification. Schedule is PRN.
Required Qualifications
- Graduate of an accredited school of professional nursing program
- Current license as a RN in the state(s) of Colorado
- Minimum of two (2) years of nursing experience
- CPR certified

- Baccalaureate degree preferred
- Home care experience preferred
