Job Summary
Assess, determine eligibility, and process admissions for patient and family caseloads, managing the full nursing process from planning through evaluation. Obtain data on physical, psychological, social, and spiritual factors to coordinate optimal care with physicians and the interdisciplinary team. Provide visits to re-assess needs, update care plans, perform therapeutic procedures, and conduct clinical pronouncements of death while maintaining accurate records. Work as-needed shifts Monday through Friday from 8 am to 4:30 pm with two days of on-call rotation monthly.
Required Qualifications
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Maintains a current CPR certification
- Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public
- Valid driver's license
- Proof of insurance
