Job Summary
Assess patients through history, testing, and caregiver observations while consulting the interdisciplinary team. Teach patients and families health care regimens, disease management, and wellness plans. Safely administer medications and treatments as ordered, managing refills with provider authorization. Complete biannual and status change nursing assessments for an assigned panel, documenting problems, goals, and interventions in care plans. Utilize assessment tools including the PACE Nurse Assessment and Medication Management to collect data. Requires a Baccalaureate degree in nursing, current Massachusetts license, and BLS certification. Minimum of two years experience in ambulatory, home health, or sub-acute settings preferred, plus one year with a frail or elderly population. Pay range $40 up to $60 based on experience, with a $10,000 sign-on bonus. NeighborHealth serves a diverse demographic across all cultures and backgrounds.
Required Qualifications
- Current Massachusetts license in nursing
- Current BLS certification
Desired Qualifications
- Baccalaureate degree in nursing preferred
- Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred
- A minimum of one year experience working with a frail or elderly population
- Prior experience in interdisciplinary team setting a plus
