RN Primary Nurse (PACE)

Job Summary
Oversee, assess, plan, and construct nursing care for participants in the PACE program. Obtain patient histories, perform tests, and consult with the interdisciplinary team to collect observations. Teach patients and families about health regimens, medications, diet, and wellness while safely administering treatments and managing medication refills. Complete biannual and status change nursing assessments for assigned panels, documenting problems, goals, and interventions in care plans using PACE assessment tools. Requires a Baccalaureate degree, current Massachusetts license, BLS certification, and experience with elderly populations.
Required Qualifications
- Current Massachusetts license in nursing
- Current BLS certification
- Minimum of one year experience working with a frail or elderly population
Desired Qualifications
- Baccalaureate degree in nursing preferred
- Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred
- Prior experience in interdisciplinary team setting a plus
