Job Summary
Conduct day-to-day nursing assessments and deliver quality patient care with comprehensive documentation. Manage integrated medical, rehabilitation, case management, and adult day health services within the PACE Center. Maintain current MA RN licensure with at least one year of experience in ambulatory, home health, hospital, or sub-acute settings. Prioritize working with a frail or elderly population, with interdisciplinary team experience highly valued. Work Monday through Friday, 8:00 AM to 5:00 PM, including weekend, holiday, and on-call rotations.
Required Qualifications
- Current MA RN License
- Monday - Friday, 8:00 AM - 5:00 PM schedule
- Weekend/holiday/on-call rotation
Desired Qualifications
- Minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting
- Minimum of one year experience working with a frail or elderly population
- Prior experience in interdisciplinary team setting
