Job Summary
RN Primary Care Navigator will coordinate chronic disease management and patient education via telephone, enrolling patients in the CCM Program, educating on program benefits, and facilitating timely scheduling of appointments, referrals, and tests. The role acts as a liaison among patients, families, and healthcare teams to ensure continuity of care, supports access to resources (including housing and care assistance), and collaborates with multidisciplinary teams to optimize outcomes. The position requires Montana RN licensure (or multi-state compact with MT authorization), BLS certification, strong organizational and communication skills, proficiency with basic software and EHR systems, and a track record of working autonomously in a patient-centered care environment.
Required Qualifications
- Montana RN license (or multi-state compact license with MT authorization)
- BLS certification
- Ability to use computer software (Word, Outlook, Excel, PowerPoint, Access) and EHR familiarity preferred
- Strong communication skills and ability to work autonomously
- Experience with chronic disease management and care coordination preferred
- Bachelor’s Degree in Nursing preferred
