Ambulatory nurse role: Provides care to individuals, families, and communities within the RN scope of practice; performs assessment, planning, implementation and evaluation; ensures complete and accurate documentation; delivers health and wellness education; serves as preceptor and supports development of other staff and healthcare students; provides leadership for quality-improvement initiatives; assists in developing departmental standing orders; engages in registry work for PCMH follow-up nurse visits, phone triage, and transitional care management.
