RN - Physician Practice - Full Time - Internal Medicine

Provide professional nursing care to patients while supporting the physician practice network with patient education, triage, and documentation. Screen and document patient calls to determine urgency, retrieve records, and file or import test results per provider direction. Conduct nurse visits, review CC'd charts, and communicate with providers or patients as needed for referrals and medication changes. Prioritize patient care by implementing physician orders, monitoring laboratory and imaging data, and ensuring patients receive appropriate instruction on diagnoses and wellness. Collaborate with the team for daily huddles and care transitions.
