Job Summary
RN Patient Transportation Coordinator oversees coordination of services to facilitate seamless transitions from acute and long-term care to Home Care. Responsibilities include clinical review of data and referral acceptance based on established criteria, coordinating services and information dissemination, following referrals through to completion, serving as a primary information resource about RHCD services, and providing leadership and supervision to RHCD Patient Transition Coordinators. Requires RN licensure in Virginia, completed nursing education, CPR/BLS, and prior nursing/home health/care-management experience. The role emphasizes customer service, data accuracy, and ongoing knowledge of Medicare/insurance criteria and RHCD products/services.
Required Qualifications
- Education Program Graduate, Nursing (Required)
- Associates Degree, Sociology/Social Work/Nursing (Required)
- Bachelors Degree, Sociology/Social Work/Nursing (Preferred)
- Experience 1 year Nursing experience (Required)
- Home health experience (Preferred)
- Case management and health care delivery experience (Preferred)
- Licenses and Certifications Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
