Job Summary
Oversee all patient admissions and transfer activity while monitoring capacity currently and prospectively. Facilitate patient referrals from outside the local area and ensure continuous throughput from all entry points using effective verbal communication and swift decision-making. Serve as a liaison between clinical departments to secure the best possible placement for all patients within resource constraints. Facilitate PACE on-call after hours, weekends, and holidays to support community services, and participate in Remote Home monitoring by managing patients and on-call services. This role supports continuity of operations within the facilities.
