RN - Patient Navigator

Navigate patients through the continuum of care by triaging, coordinating, and managing patient care as a resource person for the multidisciplinary team. Educate patients and families on condition status, care goals, and the cycle of care while serving as a liaison between patients, providers, and support services. Proactively assess patient needs prior to arrival, expedite clinic appointments, and interact with clinical staff, social workers, and financial counseling to ensure integrated care. Participate in quality improvement processes to enhance program operations and patient satisfaction while developing systems that organize patient flow from referral through treatment.
