Coordinate patient navigation through diagnosis, treatment, and follow-up for cancer care programs, ensuring seamless transitions between care settings. Act as a resource to interdisciplinary teams and families by facilitating efficient care, providing education, addressing psychosocial needs, and identifying barriers to support optimal outcomes. Develop customized plans for each patient, manage resource allocation, and utilize data to close healthcare gaps and achieve consistent clinical results. Collaborate with multidisciplinary teams to discuss cases, design patient education materials, and assist in developing program goals and objectives. Work 40 weekly hours at the Rapid City location with a starting pay range of $32.00 - $40.00.
